Care workers should not be framed as conduits for control. They are often the first people forced to translate a rule they did not write, under staffing pressure, with confused families in front of them.

Map the architecture in plain columns: requirement, penalty, exemption, appeals route, enforcement office.
The penalty matters because it tells you whether the system is asking, warning, or compelling. The exemption matters because it shows who the designers imagined as legitimate.

For preservation: keep the blank form, one completed sample with personal data removed, the envelope or notice header, and any revision marks.
Note paper size, code numbers, and whether the instruction was posted, mailed, or handed out. Do not clean the artifact so much that the bureaucracy disappears.

The form is not a neutral container. It is the machine.
It decides what counts as an event, who can speak, what must be timed, and which omissions become suspicious. The condition enters as circumstance, but the form converts it into obligation.

Keep H1, H2, and H3. Do not add H4 unless the documents force it.
If every case uses the same three headings, comparisons stay possible across towns, agencies, and years. The discipline is dull on purpose.
